All assignments given to A-United must be accompanied by an A-United request form. The form must be attached to your documents at the time of pick up/fax. All items that do not have a proper work order request form attached to it will not be processed.

A-United Court Appearance Request
This form must accompany all requests for A-United to provide your office with a special appearance attorney.

A-United Court Service Request
This form must accompany all requests for A-United to file your documents at the courthouse.

A-United Court Research Request
This form must accompany all requests for A-United to obtain copies of items for you at the courthouse.

A-United Investigative Service Request
This form must accompany all requests for A-United to investigate individuals, companies or subjects.

A-United Messenger Service Request
This form must accompany all requests for A-United to deliver items to specified locations.

A-United Photocopy Service Request
This form must accompany all requests for A-United to photocopy or scan documents at a given location.

A-United Process Service Request
This form must accompany all requests for A-United to effectuate service upon an individual or company.

Los Angeles County Recorders Form
In addition to your normal work order request, you must also attach this form for all items that need to be recorded at the Los Angeles County Recorder's Office. They will not accept any recordings without it.
